Gather Supporting Evidence

Comprehensive documentation is needed to support an SDA application. This typically includes reports from allied health professionals (particularly occupational therapists), evidence of the current housing situation and why it's unsuitable, information about support needs and how they're currently being met, and details about goals and how SDA will help achieve them.
A Functional Capacity Assessment from an occupational therapist is particularly important, as it provides detailed evidence of daily living challenges and housing needs
Step 3
Submit a Request or Plan Review
- SDA funding can be requested during a plan review or by submitting a Change of Circumstances request if the situation has changed significantly.
