Support Coordination
At Enlarge Living we have a dynamic team of experienced and professional Coordinators who are innovative, creative, and driven to provide you comprehensive support tailored to your individual needs.
We focus on building capacity: “doing with, not for” to empower participants to take control of their lives and achieve outcomes.
We continually review our program as we grow and expand the expertise of our team.
This allows our team to provide participants from diverse backgrounds support to pursue and achieve their goals through the utilisation of their NDIS funding, mainstream services and community engagement.
There are 3 levels of support coordination that can be included in a participant’s plan
Support Connection
- Support to help you understand your plan, connect with NDIS providers and community, mainstream and other supports to get the most out of your NDIS plan.
- It should also increase your confidence and skills to manage your plan independently.
Coordination of Supports
- Support to put in place a mix of supports to increase capacity to maintain relationships, manage tasks, live more independently, and be included in your community.
- It builds the confidence and skills to direct your lives, not just your services.
Specialist Support Coordination
- A Specialist Support Coordinator has a greater degree of qualifications and skills to provide a multifaceted level of support for participants whose situations are more complex.
- A Specialist Support Coordinator will assist participants to overcome and manage the challenges and barriers in their support environment and ensure a consistent delivery of service.
Your NDIS plan may describe what level of support coordination is funded or how your support coordination should be delivered, and this means you may only purchase support coordination in the way described. If the level of support coordination funding is not described in the plan, you can choose what level of support coordination is most appropriate for your needs and how it’s delivered.
Our Support Coordinators can assist with:
- Understanding your plan funding and how it can be used
- Identifying and discussing a range of available NDIS, informal, community and mainstream supports to ensure you are well supported
- Support you to exercise your choice and control when choosing and engaging providers
- Support to build your confidence and skills to work towards achieving your goals
- Building your capacity and understanding of how to navigate NDIS, manage your funds and services
Are you looking for a Support Coordinator that will:
- Take steps to raise and act on concerns about matters that may impact the quality and safety of supports and services provided to you
- Take all reasonable steps to prevent and respond to all forms of violence against, and exploitation, neglect and abuse of, people with disability
- Act with respect for individual rights to freedom of expression, self-determination, and decision-making in accordance with relevant laws and conventions
- Provide supports and services in a safe and competent manner with care and skill
- Act with integrity, honesty, and transparency
